  1. The FCC's current Form 610 license application dated
  2. March 1992.

  4. This new 610 form must be used for all applications
  5. which include a request for 13 or 20 WPM Morse code exemption.

  8. USING THE 610 FORM:
  9.

  11. LOST LICENSE
  12.  
  13. If you have lost your license, be aware that VECs are not 
  14. permitted to submit any upgrades to the FCC without a copy 
  15. of your current license attached to the 610 form.  In order to 
  16. expedite your upgrade, you must write the FCC and request a 
  17. replacement license.  Since that can take several weeks to 
  18. arrive, we recommend that you also request a Form 1010-B 
  19. verification letter from the FCC which will arrive in a 
  20. couple of weeks--and is legally binding license document.  
  21. You may use that form as a license until your Form-660 
  22. license arrives.

  24. The original and a copy of Form 1010-B must be brought to the 
  25. test session verifying that you are currently licensed--
  26. the copy of which will be attached to your upgrade application 
  27. (in lieu of an actual Form-660 license copy).  To request 
  28. the Form 1010-B Verification Letter, write to: FCC, 1270 
  29. Fairfield Rd, Gettysburg PA 17325-7245; indicating that 
  30. you lost your license and that you are requesting that a 
  31. replacement license be issued to you--also be sure to 
  32. request that they send you a Form 1010-B Verification 
  33. Letter verifying that you are licensed.

  36. CALL SIGN CHANGE REQUEST

  38. Check and initial Box 2E, and complete lines 3 through 14
  39. of Section I.  You must attach a copy of your current FCC
  40. license to the top of the back of the application.

  43. ADDRESS CHANGE
  44.  
  45. Check Box 2G (and 2H if the station location has also changed) 
  46. and complete lines 3 through 14 of Section I.  You must attach 
  47. a copy of your current FCC license to the top of the back 
  48. of the application.

  51. LICENSE RENEWAL

  53. Check Box 2A and write the expiration date to the right (as directed)
  54. and complete lines 3 through 14 of Section I.  You must attach a copy 
  55. of your current FCC license to the top of the back of the application.

  58. LICENSE REINSTATEMENT (if expired less than 5 years)

  60. Check Box 2B and write the expiration date to the right (as directed)
  61. and complete lines 3 through 14 of Section I.  You must attach a copy 
  62. of your current FCC license to the top of the back of the application.

  65. NAME CHANGE

  67. Check Box 2F and write the former last and first names to the right 
  68. (as directed) and complete lines 3 through 14 of Section I.  You must 
  69. attach a copy of your current FCC license to the top of the back 
  70. of the application.

  73. TECHNICIAN CLASS GRANDFATHER CREDIT

  75. If you hold a Technician class license which was issued 
  76. prior to 3/21/87, you are not required to take the Element 
  77. 3B (General class written) examination if you can provide a 
  78. copy of your FCC-license dated before that date.  If your 
  79. licensed has since been renewed, and if you don't have a 
  80. copy of the pre-3/21/87 license, you must seek a 
  81. verification letter from the FCC.  To do so, write to FCC, 
  82. 1270 Fairfield Rd, Gettysburg PA 17325-7245 and request a 
  83. Form 1010-B Verification Letter verifying that you were 
  84. licensed as a Technician class prior to 3/21/87.  After you 
  85. receive that form from the FCC, bring it to the test session 
  86. with you in order to claim your deserved credit.
